The 76-year-old metal legend has been battling degenerative disorder Parkinson's disease since 2019 and the condition has affected his mobility.
But he's determined to reunite with his band mates for one last concert in his hometown of Birmingham, England in July.
During an appearance on Sirius XM, he explained: "I have made it to 2025. I can't walk, but you know what I was thinking over the holidays? For all my complaining, I'm still alive.
"I may be moaning that I can't walk but I look down the road and there's people that didn't do half as much as me and didn't make it."
Ozzy's wife Sharon has also insisted the singer's mobility problems don't affect his ability to perform because his voice is still as strong as ever.
She told The Sun newspaper: "He's very happy to be coming back and very emotional about this [planned gig].
"Parkinson's is a progressive disease. It's not something you can stabilise. It affects different parts of the body and it's affected his legs. But his voice is as good as it's ever been."
Osbourne has stepped away from touring in recent years as he battles spinal injuries and Parkinson's disease, but he will be back onstage for the special hometown charity gig at Birmingham's Villa Park on July 5.
The 'Back To The Beginning' show will feature a short solo set from Osbourne before he joins the rest of the group for what has been described as his "final bow".
It will mark the first time he has reunited with his band mates Geezer Butler, Tony Iommi and Bill Ward in 20 years.
"It's my time to go 'Back to the Beginning'… time for me to give back to the place where I was born,' he said.
"How blessed am I to do it with the help of people whom I love. Birmingham is the true home of metal. Birmingham Forever."
Tickets for the show will go on sale on February 14.
